Spontaneous intracranial hypotension complicated with cerebral venous thrombosis and intracerebral hemorrhage in a patient with protein S deficiency: a case report

چکیده
Cerebral venous thrombosis (CVT) has been reported as an uncommon complication of spontaneous intracranial hypotension (SIH). Since this is a rare presentation, we report a patient with SIH complicated by CVT and intracerebral hemorrhage (ICH). A 43-year-old woman presented with a 2-day history of orthostatic headache and neck stiffness. Magnetic resonance imaging (MRI) of the brain demonstrated typical findings of SIH with pituitary hyperemia and engorgement of the venous structures, as well as flattening of the pre-pontine space. Lumbar puncture confirmed low opening cerebrospinal pressure (10 mm H2O). Neurological examination on admission was normal, except for left optic nerve atrophy with associated blindness that was due to an old central retinal artery occlusion. However, on day 3 of admission, the patient developed acute left-sided hypesthesia and hemiparesis. Computed tomography (CT) scan of the brain revealed CVT related ICH. The headache failed to respond to conservative treatment, but resolved within an hour of applying an epidural blood patch to the lower cervical region. Thrombophilia screening was performed and showed protein S dysfunction (<10 %). Warfarin was initiated as maintenance therapy and the patient did not experience any recurrent CVT event or orthostatic headache during the 1.5-year follow-up period. This report highlights that CVT with ICH is a potential complication of SIH. Any change of neurological status in a patient with SIH should raise the index of suspicion for CVT. A thorough investigation of the underlying cause is warranted and should include a thrombophilia screen. * Corresponding author. Spontaneous Intracranial Hypotension Plus Cerebral Venous Thrombosis: A Case Report Study
This is a case study of a 34-year-old woman who was admitted to hospital with a history of severe orthostatic headache. She was diagnosed as having spontaneous intracranial hypotension (SIH) by undetectable cerebrospinal fluid (CSF) pressure at lumbar puncture, and with evidence of diffuse dural enhancement of the brain detected by magnetic resonance imaging (MRI). However, the contrast-enhance...

متن کاملLacunar infarction in child with Protein S deficiency: a case report
Abstract Arterial ischemic stroke defines as a new focal neurologic deficit that lasted 24 hours or longer. Stroke is relatively rare in children and incidence of cerebrovascular disease is 1 per 4000 in neonates and 1 per 7000 to 1 per 70000 in older children (1 month to 18 years). Protein S deficiency is one of the causes of the stroke in children. Major manifestations of protein S deficienc...
متن کاملCerebral venous sinus thrombosis caused by spontaneous intracranial hypotension
The association of spontaneous intracranial hypotension with cerebral venous sinus thrombosis is rare. We report here a case of extensive cerebral venous sinus thrombosis involving three sinuses following spontaneous intracranial hypotension. The patient presented no other thrombotic risk factors except for spontaneous intracranial hypotension. This case adds to the evidence that spontaneous in...

Introduction: Sudden sensorineural hearing loss (SSNHL) is an important otological emergency. Up to 90% of the cases are idiopathic. Cerebral venous thrombosis (CVT) is an extremely rare identifiable cause as it only represents 0.5% of all strokes. Case Report: In this paper, an unusual case of bilateral SSNHL secondary to bilateral CVT with rapid and complete recovery is reported. The patie...
